Terms of Service
Consultation Agreement
Appointment durations are the total time spent with your therapist and include a professional consultation which will occur before and after each treatment. Generally, 5 minutes either side of a treatment is utilised for testing and consultation purposes.
Health Agreement
You agree that all the information I have provided regarding my health history and medical conditions is accurate and acknowledge that to ensure an appropriate treatment it is of utmost importance to inform my therapist of any old, current, or new injuries, allergies and/or medications; as well as inform them of any changes in my health status and concerns that may arise. You understand that failure to do so may pose a threat to your health and physical well-being and withhold Body Align Health and the therapist from any liability whatsoever arising from failure on my part.
I have clearance from my physician to receive massage therapy.
I will inform the massage therapist of any infectious diseases/conditions I may have, including and not limited to the skin, respiratory and blood.
Treatment Agreement
You understand that the therapist only assess musculoskeletal dysfunctions and does not diagnose illness, disease, or any physical or mental disorder, nor do they prescribe medical treatment or pharmaceuticals.
You acknowledge that massage therapy is not a substitute for medical examination or diagnosis, and I will seek a physician or appropriate health professional in aid of such procedures and treatments.
The massage therapist works soft tissue through deep manipulation and may integrate tractions and range of motion exercises to the joints but will not administer spinal manipulations.
You understand that the treatment given is designed to address the care and prevention of myofascial pain and dysfunction.
If you experience any pain or discomfort during the treatment, you'll immediately inform the therapist so that adjustments to the technique and pressure can be made. You also understand that you have the right to stop the massage treatment at any time.
I understand that I or the massage therapist may terminate the session at any time.
Should the session require, after your therapist has left the room, you may disrobe to your comfort level (keep underwear on if you wish).
I understand the risks associated with massage therapy include, but are not limited to, superficial bruising, short-term/delayed onset muscle soreness and exacerbation of unknown injuries.
Professional Service Policy
I understand that Body Align Health provides only professional, health and wellbeing-focused treatments that are entirely therapeutic and non-sexual in nature. Nothing outside this scope is provided. Please address your therapist the way you would a physiotherapist or chiropractor.
Inappropriate behaviour will not be tolerated and may be prosecuted to the full extent of the law.
I have been informed that the massage therapist must meet standards and principles enforced by the Massage Therapy Act & Regulated Health Professionals Act and may at any time refer a patient elsewhere if they feel that they would have greater potential with an alternative practitioner.
I understand the therapist works from an at-home practice (residential property) within an apartment complex managed by Strata and will act according to this setting. I also waive and release Strata from all liability past, present and future relating to any injuries or incidents that may occur on this common property.
Cancellation & No-Show Policy
You’re aware that by making a booking you've agreed to the cancellation & no-show policy below:
• 100% of the total owing is required if you cancel within 24 hours of the appointment.
• No-shows will receive no refund. If no pre-payment has been received, a link will be texted to you so you can make the required payment owing.
If you cancel with short notice or miss your appointment, your therapist is left without income for that period. Calling a friend to take your appointment instead of cancelling is an excellent alternative and highly recommended.
Gift Card Policy
Gift-cards are non-refundable. Once purchased they must be treated like cash.
Privacy & Confidentiality Policy
I understand that my personal and medical details will be kept private for a minimum of 7 years and will not be shown to anyone else without my written (digital or hand) permission; unless subpoenaed by a court or the therapist considers I may be a danger to myself or others.
By making a booking you hereby agree to the Terms & Conditions found on this page and waive and release Body Align Health and the therapists from all liability, past, present, and future relating to all bodywork by this provider.
